Please be informed that the Hospital Infection Control Unit (HICU) will now implement an online validation process for the following:

CAMP – Incoming 4th Year

CD – Incoming 5th Year

CM – LU5 to LU7

CN – All Year Levels

CP – Incoming 5th Year (Hospital Pharmacy Rotation)

CPH – Incoming 4th Year

STEPS TO FOLLOW:

1. Complete all required laboratory and diagnostic tests;

2. Once all requirements are complete, fill out the HICU Google Form using the QR code;

3. Submit the form at least 2 days before your preferred UPHS Final Clearance schedule.

You will receive an email from HICU regarding the status of your clearance.

Please ensure that the information you provide in the Google Form is accurate and complete.
