Notice of Revision – Student Loan Application Form (SLB)

The Student Loan Board of UP Manila formally informs all concerned that the Student Loan Application Form has been revised, effective March 06, 2026. 

The updated version includes improvements to ensure clarity, accuracy and completeness of the required information. All future applicants are advised to use the revised form for current and future submissions.

Latest version can now be viewed and downloaded from the OSA website. Please disregard any previous versions of the forms.
