Catch-Up Schedule for Annual Physical Examination (APE)/ Periodic Health Examination (PHEX) for UPPER level students AY 2026–2027 

Please be informed that the UP - PGH Health Service will conduct CATCH-UP SCHEDULE for UPPER LEVEL students who were unable to complete their Annual Physical Examination (APE)/Periodic Health Examination (PHEX), which is a PRE-REQUISITE for enrollment for Academic Year 2026–2027. 

Catch-Up Period: June 22, 2026 – August 7, 2026 

30 slots available daily (face-to-face consultation or validation) 

Monday, Tuesday, Wednesday & Friday except holiday: 

15 slots: 7:00 AM – 10:00 AM 

15 slots: 11:00 AM – 2:00 PM 

Thursday:

 30 slots: 11:00 AM – 2:00 PM 

Important Reminders: 

• The dates and slots mentioned are open to all Colleges.
•  First come First served Basis No need to secure an appointment slip from your College. 
• Students are encouraged to arrive early to secure a slot and complete requirements prior to your preferred date for APE/PHEX clearance.
