Requirements:

Application form is obtainable at OSA and downloadable at https://osa.upm.edu.ph/node/411
Current Form 5
Parent's Income Tax Return (ITR) and/or Certificate of Tax Exemption from the BIR if parents are unemployed.
Endorsement letter from the Office of the College Secretary.

Rates: (based on the slasonline application)

Php 1,500 - 5,000 / month
